The layperson's and the professional perspective on health care Health care encounters between health professionals and patients in general can be regarded as encounters between a life-world perspective and a professional perspective. The interaction between the health professional and the layperson is often described and discussed in terms of how the perspectives of the two parties are expressed or "heard" in the encounter. The concept of "voice" Mishler, 1984, Linell, 1991 ; has recently come into frequent use to illustrate which party, or rather which perspective, is being "heard" in the encounter. By and large, there is a vast amount of evidence that, in encounters with the professional, the patient expresses her ideas on health only to a very limited extent. This basic asymmetry in the relationship between the professional and the patient can to some extent be seen as inherent in the tasks of medicine. Looming in the background of all medical encounters is the notion of the professional saving the life of the patient. Of course, this is most obvious in emergency situations or in the treatment of lifethreatening diseases, but is possibly there even in dealing with the most trivial health problems. The clinical situation is thus asymmetrical in roles, tasks and responsibilities related to an asymmetry in knowledge and access to medical information, all contributing to legitimizing the professional role. As the "special knowledge" of the professional is recognized by both parties, the layperson and the professional, this also means that social power is asymmetrically distributed. To be helped, the patient has to rely on the professional and to some extent has to surrender social power. This means that to understand what is going on in individual encounters, we must look not only at the question of different perspectives and different schemes of knowledge, but also at the question of how knowledge is socially and normatively distributed, and how knowledge is ascribed to role-identities. There is also evidence that the patient is prevented from expressing herself by the style of questioning used by the professional, by her feelings of insecurity or inferiority in conversations with the professional or by having a different way of defining the health issues and thus what is relevant to take up Kleinman 1980, Fisher & Todd 1983, Mishler 1984, Sachs 1989, Aronsson 1991, Linell 1991.
