FamciclovirTzanck testing is an easy, inexpensive technique to confirm suspicion of AHGS, and it has the added advantage that it can be performed in the office. The reported sensitivity is 40% to 50%, depending on interpreter experience and degree of maturation of the viral vesicle pustules or ulcers tend to yield lower sensitivities ; . When in doubt, patients can be referred to hospital for viral culture or direct immunofluorescence, although such referral must be done soon after presentation, because of the self-limiting course of AHGS. If gingival hyperplasia is present without oral vesicles or ulcers, the workup should include routine blood testing complete blood count with differential count and peripheral blood smear ; to rule out any abnormalities suggestive of leukemia. Conventional antiviral therapy associated with oral HSV has been acyclovir in either cream or oral form ; . However, because of its poor gastrointestinal absorption and bioavailability, acyclovir has not routinely been used in the management of AHGS except for the oral suspension administered in a rinse and swallow technique.1 Valacyclovir and famciclovir are 2 more recently developed antiviral agents that may be used in the treatment of AHGS. Valacyclovir is an altered form of acyclovir, which acts by increasing, by 3 to 5 times, the bioavailability of acyclovir to which it is converted via hepatic metabolism ; .8 It is well tolerated in healthy patients and is prescribed in doses of 1 g tid for 7 days for herpes zoster, although 1 g bid should be effective for AHGS. Famciclovir, the oral prodrug of penciclovir, has an oral bioavailability 3 to 5 times that of acyclovir. Both penciclovir and acyclovir function through competitive inhibition of viral DNA synthesis by means of selective phosphorylation by viral thymidine kinase. Although acyclovir is a more potent inhibitor of viral DNA polymerase, the advantage of penciclovir and its analogues is that it is present in infected cells at much higher concentrations and for longer periods than acyclovir and its analogues.9 Penciclovir is marketed only in the United States, in a topical form cream ; . However, famciclovir is available in Canada and has been successfully used by the authors of this review. The dosage of famciclovir recommended for treatment of herpes zoster is 500 mg tid po for 7 days, although 500 mg bid was effective in AHGS. A significant acceleration in clinical resolution can be seen with the use of anti-viral therapy. The earlier these medications are given, the more effective they are. They do not affect dormant virus protected in nerve ganglions and therefore will not eliminate the virus completely. After treatment of a primary infection a patient may still experience episodes of recurrent herpes labialis if the virus becomes reactivated. The severity and quantity of intraoral lesions may significantly reduce dietary intake and predispose the patient to dehydration. Thus, it is important to balance any decrease in intake with fluids. The results of this investigation show increases in reaction times in patients and controls during all vestibular stimulation conditions and during ocular pursuit. There were also significant interactions of group and IPT as well as group and vestibular stimulation. Patients with unilateral vestibular dysfunction showed greater decrements in information processing during vestibular stimuli than controls. These results reveal interference between vestibulo-ocular processing and a concurrent reaction time task, suggesting the VOR and ocular motor system are dependent upon cognitive resources to some extent and are not fully automatic systems. N June 2004, a full-page advertisement in The New York Times criticized the American Veterinary Medical Association for not taking a stronger position on the management of pregnant sows, veal calves, and laying hens. This illustrates the need for the veterinary profession to have clear positions on questions of animal care. In recent years, veterinarians in the United Kingdom and the US have indicated, through their professional journals and in personal communications, their belief that the profession's commitment to serving clients has overridden the profession's responsibility to question some animal welfare issues in existing systems. It is notable that veterinarians are largely not involved in the search for workable solutions to these complex, value-laden questions. However, veterinarians' growing concern is encouraging, and may be expected to lead to the appropriate contribution by the profession as a whole, and by individuals as researchers and clinicians and flavoxate.

Objectives: The aim of this study was to compare changes in Doppler ultrasound studies of the fetal circulation in normal pregnancies with a group of pregnancies complicated by insulin-dependent diabetes mellitus. Study Methods: In addition to the non-stress test, fetal biophysical profile, Doppler measurements of the fetal circulation included the umbilical artery UA ; resistance index RI ; and the fetal middle cerebral artery MCA ; resistance index RI ; . Altogether 86 pregnancies complicated by insulin-dependent diabetes mellitus constituted the study group. 86 uncomplicated pregnancies with a term delivery of an appropriately grown baby were used to define normal range. 23 pregnancies with diabetes mellitus type I were complicated by preeclampsia. Doppler studies of umbilical artery and fetal middle cerebral artery velocity waveforms and biophysical profile were performed at 30-40 weeks of gestation. Data were analyzed with regard to asphyxia and otherwise complicated fetal outcome. Results: Fetuses of diabetic women displayed changes in umbilical artery flow and fetal middle cerebral artery flow in 15 cases complicated with preeclampsia and in 4 cases with intrauterine growth retardation. Conclusions: Doppler flow velocimetry of the fetal middle cerebral artery and umbilical artery together with biophysical profile and nonstress test has a high significance in predicting adverse fetal outcome, especially in the growth-retarded fetus. Diffuse myometrium hemangioma is a rare lesion which belongs to the uterine vascular tumors. Although the diagnosis is histological, it can be suspected by ultrasonographic examination. The authors report a case of a 46-year-old woman with post-partum progressive dysmenorrhea eight years ; . Physical examination detected only a little enlarged uterus. Ultrasonography with color-Doppler revealed the uterine anterior wall enlarged containing a great amount of vascular channels, some of them with low resistance blood flow. Hypothetical diagnosis of uterine vascular tumor versus angiosarcoma was made. Total hysterectomy with bilateral salpingo-oophorectomy was decided and performed with success. The final pathological diagnosis revealed a rare case: a diffuse myometrium hemangioma. One year after surgery the patient had no complaints.
